Amecrin ej tillgängligt!

Martin Höglund har meddelat SFH att amekrin fn ej är tillgängligt någonstans i världen. Preparatatet beräknas vara "åter på marknaden slutet av första kvartalet 2015". Detta beror på kvalitetsproblem hos den tidigare brittiske producenten av amekrin som uppmärksammades vid inspektion av engelska myndigheter i början av året. NordMedica informerade läkemedelesmyndigheter, inkl. Svenska Läkemedelsverket, om problemen för första gången i månadsskiftet mars-april. Beslut om "indragning till vidare" kommunicerades till Svenska LMV i mitten av juni. Ny, kommande producent är en väletablerad tysk firma. NordMedica har en långsiktig amibition att tillhanadahålla Amekrin. Man kommer därför att försöka kontraktera ytterligare en produktionsfacilitet för att ha som back-up vid framtida produktionsstopp.

Per Ljungman som är vetenskapligt råd i Läkemedelsverket kommer att kontakta LMV. Per har dock meddelat att tyvärr finns inga legala möjligheter för LMV att agera enligt gällande lagstiftnng. Läkemedelsförsäljning och distribution är enligt EU-regelverket en fri marknad och inget företag är legalt skyldiga att tillhandahålla läkemedel på svenska marknaden.

Efter diskussion med SFH har Martin och Helene Hallböök tillskrivit EHA för att be dem om hjälp i frågan. Du kan se texten nedan;

Dear Professor Chomienne (chair EHA),

We are chairpersons of the national Swedish AML (MH) and ALL groups (HH), respectively, and writing to draw your and EHA’s attention to the sudden unavailability of the anti-leukemic drug amsacrine for a period of (at least) nine months due to quality problems in the production process at the contracted British manufacturer. In Sweden, this decision was communicated by the pharmaceutical company NordMedica to the haematological community last week, indirectly (!), by a letter (attached) sent to the hospital pharmacies. According to NordMedica, the problems leading to the loss of production has been known since at least the beginning of this year and was, for the first time, communicated to the health authorities in March 2014.

Amsacrine is an "old" drug but still very useful in the treatment of acute leukaemia. In Sweden, as well in other countries, amsacrine based regimens are since many years used for treatment of patients with AML not responding to first-line chemotherapy with a "conventional" antracycline based combination (e.g. DA). At least in Sweden, it is also used in patients with AML having already received the maximal cumulative dose of antracyclines, or in those with a decreased cardiac function but still eligible for intense chemotherapy. In ALL, amsacrine is for example included in the recommended upfront treatment for Philadelphia-positive patients up to 60 years.

In summary, the sudden unavailability of amsacrine for a period of (at least) nine months is unacceptable as it may lead to suboptimal treatment in a number of patients with AML and ALL. In addition, we are critical to the way and in particular the short notice this decision has been communicated (at least in Sweden) to haematologists responsible for the treatment of patients with acute leukemia.

As representatives of a single country, our impact versus pharmaceutical companies and health authorities is very limited. Therefore, we suggest that EHA, in the interest of our patients, act by putting pressure on the company (NordPharma) to solve the production problem as rapid as possible and to take measures to avoid a similar situation in the future. Moreover, we suggest that EHA contact the health authorities to draw their attention to the problem, including a plea to give priority to the upcoming application from the contracted new manufacturer of amsacrine.
